Job summary

Banner Hospice of Northern Colorado is seeking a Certified Patient Care Assistant to provide 1:1 patient care in a home health hospice setting. This full-time day-shift role offers consistent hours (Mon–Fri, 8:00am–5:00pm) and the opportunity to build meaningful relationships with patients and families while supporting comfort and quality of life.

Qualifications

• High school diploma or GED.
• Current BLS certification.
• Current CNA in state of practice.
• Experience in medical clerical tasks preferred.
• Good communication and organization skills.

Responsibilities

• Provide routine patient care to promote comfort and support the patient’s plan of care.
• Collect vital signs, input/output, and other data and document in medical records.
• Transcribe and process physician orders in the electronic medical record as needed.
• Assist with activities of daily living, mobility, and nutritional intake.
• Perform basic procedures under supervision (e.g., vital signs, specimen collection).
• Support unit operations with clerical duties and chart management.
• Ensure patient records are complete and accurately filed.
• Assist with patient transport and coordination with care team.
• Maintain supplies and equipment and report needs to supervisor.
