Job summary

Banner Home Health is seeking a Patient Care Assistant in the Banner Hospice of Northern Colorado area. This role focuses on 1:1 patient care in a home health setting, offering day-shift hours and a full-time schedule (Mon-Fri, 8:00am–5:00pm).

The ideal candidate will hold a CNA certification, current BLS, and a high school diploma or GED, with strong organizational and communication skills to support patients and families through end-of-life and palliative care in a hospital-adjacent home

Qualifications

• High school diploma or GED required.
• Current BLS certification required.
• Current CNA certification in state of practice required for new hires.
• 1 year medical clerical or related experience preferred.

Responsibilities

• Provide routine patient care to promote comfort and contribute to the care plan.
• Collect vital signs and other delegated measurements; document in EMR.
• Communicate patient status clearly to the care team to ensure continuity of care.
• Perform unit support tasks and basic clinical procedures under supervision.
