Earn $25.00-$35.00 per hour, with 1:1 nurse-to-patient ratios, comprehensive benefits, and opportunities for online training and CEUs. Minimum PA LPN license and six months home care experience required; CPR and reliable transportation essential.

Qualifications

• Graduate of an accredited practical nursing program with active PA LPN license.
• Six months experience in home care or healthcare setting.
• CPR certification.
• Reliable transportation.
• Strong communication skills and compassion.

Responsabilités

• Medication administration and nutrition via g-tube
• Manage and monitor tracheostomy
• Work with a team to deliver quality patient care
• Take patient vitals, do assessments, document progress and report changes to RN supervisor
• Assist with personal care tasks such as bathing, dressing, grooming, and oral hygiene
• Follow the individual’s plan of care
• Collaborate with interdisciplinary team to coordinate patient care
