Job summary

Interim HealthCare of North Central Pennsylvania in Milton is seeking a Home Health LPN to provide one-on-one patient care in a home setting. You’ll administer medications, monitor vitals, assist with daily living activities and follow individualized care plans.

Earn $25.00-$35.00 per hour, with 1:1 nurse-to-patient ratios, flexible assignments, ongoing online training, CEU opportunities and benefits including health coverage and a 401(k).

Qualifications

• Graduate of an accredited practical nursing program with PA LPN license.
• Six months experience in home care or healthcare setting.
• CPR certification.
• Reliable transportation.
• Strong nursing proficiency and compassionate communication.

Responsibilities

• Administer medications and nutrition via g-tube.
• Monitor and manage tracheostomy.
• Collaborate with the team to deliver quality patient care.
• Take vitals, assess, document progress and report changes to RN.
• Assist with activities of daily living (bathing, dressing, grooming).
• Follow the individual’s plan of care.
• Coordinate care with interdisciplinary team.
