Job summary

LINKEDCORP HR CONSULTANCY PTE. LTD. seeks a Social Worker to work with youths and families in the community, delivering meaningful interventions and reintegration support through collaborative case management.

You will partner with Probation Officers, CPOs, schools, families, and employers, develop tailored intervention plans, and document progress in line with organizational standards.

Qualifications

• Bachelor’s degree in Social Work or a related field.
• At least 2 years of relevant experience in social services or youth-focused environments.
• Familiarity with clinical social work practices and a desire to develop therapeutic skills.
• Enthusiastic about working with at-risk youth and contributing positively to their reintegration.

Responsibilities

• Individualized case management and intervention planning.
• Collaborate with Probation Officers, CPOs, schools, families, and employers for joint case management.
• Develop partnerships with schools and social service agencies to support reintegration.
• Present case progress at meetings and review platforms.
• Maintain accurate case notes and documentation per guidelines.
• Coordinate with Hostel Operations for scheduling and movement planning.
• Lead or participate in group-based interventions for youth residents.
• Support ad-hoc tasks as assigned by supervisors or management.
