Job summary

The Supreme HR Advisory Pte. Ltd. is seeking a social services professional to manage individualised case plans and drive reintegration efforts for residents at Buangkok.

The role requires coordinating with probation officers, child protection officers, families, schools and employers to develop and review intervention goals. Responsibilities include conducting casework, documenting records, and leading group activities, with ad-hoc duties as needed.

Qualifications

• Degree in Social Work or equivalent with 2 years of experience in social service sector or similar setting.

Responsibilities

• Independent case management for intervention goals from onset and timeline reviews.
• Collaborate with Probation Officers (POs), Child Protection Officers (CPOs), families, schools and employers.
• Develop partnerships with stakeholders and CSO agencies for reintegration efforts.
• Prepare for, attend and present cases at platforms to update on intervention progress.
• Casework documentation for all case related records.
• Coordinate with Hostel Operations to schedule daily movement for residents.
• Conduct and participate in group work for residents.
• Support in other ad-hoc duties assigned as needed.
