Job summary

MINDS in Singapore seeks a Transition Support Officer to provide hands-on guidance to clients during transitions, partnering with a multi-disciplinary team. You will support the IDP process, assist families, and help transfer skills to community settings.

The role emphasises holistic, person-centred care and requires collaboration with AHPs, SWs and caregivers to enable smoother transitions for clients with intellectual disabilities.

Qualifications

• Diploma in Social Work, Psychology, Community Services or related field with at least 2 years working experience.
• Strong team-player with excellent people skills, organisation and time management.
• Ability to speak an additional local language besides English is an advantage.

Responsibilities

• Support the Hub Manager in development, review and update of clients' IDP using a person-centred approach.
• Coordinate Transition Planning meetings, Case Reviews and IDP meetings with AHPs and stakeholders.
• Ensure progress on IDP goals aligns with client long-term aspirations and interests.
• Collaborate with Social Workers to engage caregivers and transfer skills to home/community.
• Coordinate family meetings to review goals and longer-term plans.
• Provide hands-on training and document clients' transition training progress and adaptations.
• Advocate for clients' transitional and developmental needs with multi-disciplinary teams and partners.
• Document engagements and build a resource bank of person-centred practices for practitioners.
• Review and evaluate IDP to ensure sustainable development and track progress with stakeholders.
