Job summary

MINDS is seeking a Transition Support Officer in Singapore to provide practical support and guidance during transitions from school to community hubs and from residential to community care. You will work with a multi-disciplinary team to develop and review individualized plans for clients with intellectual disabilities, ensuring person-centred development across transitions.

Qualifications

• Minimum Diploma in Social Work, Psychology, Community Services or other relevant fields.

Responsibilities

• Support the Hub Manager in developing, reviewing and updating IDP through a person-centred approach.
• Coordinate and participate in Transition Planning meetings, Case Reviews and IDP meetings with AHPs and stakeholders.
• Ensure IDP goals align with long-term aspirations and client interests.
• Engage clients and caregivers in discussions on transitions and training needs as needed.
• Coordinate family meetings to review the IDP and longer-term goals.
• Provide hands-on training and document client progress and adaptations.
