Job summary

RN Care Pte. Ltd. in Singapore seeks a caring Social Worker to conduct assessments, plan interventions, and manage cases for clients with mental health conditions and dementia.

You will provide home visits, caregiver support and psychoeducation, and facilitate cognitive stimulation activities tailored to each client. You will collaborate with hospitals, SSAs and community partners to strengthen mental health services.

Qualifications

• Diploma in Social Work, Counselling, Psychology, Healthcare, Community Care or a related discipline.
• Preferably 2-3 years of experience in healthcare, community care or social services, particularly supporting mental health clients and caregivers.
• Strong interpersonal and communication skills, with good organisational skills and the ability to work independently and as part of a multidisciplinary team.

Responsibilities
