Job summary

The Supreme HR Advisory Pte. Ltd. in Singapore is seeking an experienced social worker to manage individual cases, develop intervention plans and review progress with timelines.

You will work closely with Probation Officers, Child Protection Officers, families, schools and employers to support reintegration. Responsibilities include coordinating with hostel operations, preparing and presenting cases at multiple platforms, and documenting all case records.

Qualifications

• Degree in Social Work or equivalent.
• 2 years of experience in social service sector or similar setting.

Responsibilities

• Individualised case management, including orientation and goal development.
• Collaborate with Probation Officers and Child Protection Officers for co-case management.
• Develop partnerships with employers, schools, CSOs for reintegration.
• Prepare for, attend and present cases at platforms to update progress.
• Document case records and maintain files.
• Liaise with Hostel Operations & Development to schedule daily movement.
• Conduct and participate in group work for residents.
• Support ad-hoc duties as needed.
