Job summary

Sembawang Family Service Centre in Singapore is seeking a dynamic social worker to support families and individuals through information, referrals, casework, and counselling. The role covers intake assessments, case management, group work, and community programmes to meet client needs.

The successful candidate will develop intervention plans, document outcomes, supervise staff and interns, and collaborate with community stakeholders to drive service improvements and advocate for resources.

Qualifications

• Conduct intake assessments.
• Refer clients to relevant agencies.
• Manage assigned cases and case referrals.
• Develop and review intervention strategies.
• Document and evaluate case outcomes.
• Complete case recordings and update client information in the case management system.
• Provide supervision, guidance and advice to support staff, volunteers and student interns.
• Plan, develop and implement relevant programmes to respond to the needs of clients.
• Advocate for appropriate resource allocation in the community.
• Network and collaborate with other community stakeholders.
• Evaluate and document programme outcomes.
• Identify trends, gaps or changes needed in provision of Centre’s services in the community.
• Undertake any other responsibilities and duties assigned.

A. Information & Referral

B. Casework and Counselling

• i. Manage assigned cases and case referrals.
• ii. Conduct assessment, planning and intervention for cases.
• iii. Develop and review appropriate intervention strategies.

• v. Document and evaluate case outcomes.
• vi. Complete case recordings and update client information and documentation in the case management system.

C. Group Work and Community Work
