Job summary

Club HEAL in Singapore seeks a Social Work Associate to engage clients and caregivers through activities designed to stimulate cognition and communication, under the guidance of the Team Lead. You will help plan and implement CREST services and monitor client progress across individual and group settings.

You will also provide daily administrative support, organize resources, coordinate outreach events with partner agencies, and ensure timely data collection and reporting to uphold program

Qualifications

• Diploma in social work, counselling, psychology or other fields related to healthcare and community care.
• 2-3 years relevant working experience in healthcare/community care.

Responsibilities

• Engage clients and caregivers through meaningful activities (cognitive stimulation, reminiscence, memory, linguistic and other appropriate activities) with supervision.
• Provide administrative or logistical support, coordinate outreach and engagement events with community partners, and generate quarterly CREST reports.
• Organise resources and logistics to ensure smooth CREST service delivery; coordinate team training and ensure attendance at sessions.

Job description

Overview of Responsibilities

The Social Work Associate is responsible for conducting the engagement and preventive activities to the clients and/or caregivers in an individual or group-based setting.

He/She will be working closely with the Social Worker (Team Lead) and Social Worker/ Case Worker in the planning and implementation of these activities, as well as to monitor clients’ condition and progress.

He/She also provides the daily administrative and operational support to the team, for the purpose of CREST service development and implementation.

He/She organises the logistics &/or resources to ensure the smooth running of CREST services. The Social Work Associate may also be involved in coordinating outreach events and activities, where required.

Key Responsibilities:

The major responsibilities of this position include, but are not limited to:

A. Programme Development Implementation:

1. Provide support to clients and/or caregivers through engaging them in meaningful activities such as cognitive stimulation activities or structured activities e.g. reminiscence, memory, linguistic and other appropriate activities) based on clients’ interest.

B. Administration and Staff Development:

1. Provide administrative or logistical support to the team in running client-related support and/or other CREST activities.
2. Assist in coordinating and planning outreach and engagement events with other community partners and agencies.
3. In-charge of timely data collation and quarterly report submission, as well as generate reports, in accordance with the CREST programme.
4. Store and maintain data and documents, in accordance with the CREST programme requirement, as well as the prevailing personal data protection and security guidelines.
5. Coordinate the team training schedule, in consultation with the Social Worker (Team Lead).
6. Ensure attendance in training and supervision sessions, where relevant, as part of the staff competency required by the CREST programme.

C. Other Duties:

1. Prepare the necessary resource material to be provided to clients and caregivers.
2. Any other duties as assigned by Reporting Officer.
