Recruit Express Pte Ltd in Singapore seeks a Case Associate to support the CREST team in outreach events and monitor client well-being, especially for those at risk of mental health issues or dementia. You will help link clients to appropriate services and resources.

Working closely with regional health services, volunteers and community groups, you will organise activities to raise awareness, conduct screenings, and document outcomes while following prescribed care plans.

Qualifikationen

• Min Diploma in Social Service, Psychology or Counselling.
• Eligible to support weekend outreach events on occasion.

Aufgaben

• Provide care coordination and service linkage to clients at risk of mental health issues and/or dementia, with resources and caregiver support.
• Plan and conduct outreach to the community and stakeholders to promote mental wellness and mental health awareness.
• Plan and conduct targeted case finding/screening to promote early identification and timely support.
• Plan and organize preventive activities and/or programmes for the community, those at risk of mental health conditions and clients.
• Conduct regular visits to monitor the well-being of the client.
• Support in initial assessment of walk-ins and referral cases, follow up with intervention plans for clients as per the prescribed care plan.
• Involve in the evaluation and documentation of the programme.
• Any other duties as assigned by reporting officer or Head of Community Social Services.

The Case Associate is responsible for supporting the team during outreach events, and assisting to monitor the well-being of clients assigned

As part of the CREST team, you will be responsible to support clients with or who are at risk of mental health issues and/or dementia by linking them to the necessary support and resources.

Monitor and support clients in the community as assigned by the lead.

For organizing and conducting outreach to increase mental health and dementia awareness and promote early identification.

Work with regional health services, Hospitals, volunteers, grassroots, schools, internal organisation departments etc. on activities and programmes for the seniors.

Essential Duties and Responsibilities:

• Provide care coordination and service linkage to clients with or who are at risk of mental health issues and/or dementia, with the appropriate resources and support including caregiver support.
• Plan and conduct outreach to the community and stakeholders including faith-based, corporate and community organisations to promote mental wellness and mental health awareness.
• Plan and conduct targeted mental health and dementia case finding/screening to promote early identification and facilitate timely support.
• Plan and organize structured preventive activities and/or programmes for the community, those at risk of mental health conditions and clients.
• Conduct regular visits to monitor the well-being of the client.
