Job summary

The Lentor Health Nursing Home (Hougang) seeks a passionate Social Care Programme Executive to design and deliver meaningful programmes that enrich residents’ lives. You will work with our nursing, rehabilitation and community teams to create engaging activities promoting social connection, emotional well‑being, cognitive stimulation and active ageing.

Qualifications

  • Diploma in any relevant disciplines required.
  • Experience in senior care, healthcare, social services, or volunteer management is preferred.
  • Ability to plan engaging social care programmes for seniors.

Responsibilities

  • Plan and conduct social care programmes for residents.
  • Design activities for social, emotional, cognitive, and functional well‑being.
  • Coordinate outings, community visits and external collaborations.
  • Recruit, orientate and coordinate volunteers.
  • Maintain programme records and evaluation reports.

About the Role

At Lentor Health Nursing Home (Hougang), we believe that every senior deserves to live with dignity, purpose, and joy.

We are looking for a passionate and energetic Social Care Programme Executive to design and deliver meaningful programmes that enrich the lives of our residents. You will work closely with our nursing, rehabilitation and community teams to create engaging activities that promote social connection, emotional well-being, cognitive stimulation, and active ageing.

Key Responsibilities
Programme Planning & Implementation
  • Plan, organise, and conduct individual and group-based social care programmes for nursing home residents.

  • Design activities that support residents’ social, emotional, cognitive, and functional well-being, such as:

  1. Arts and crafts

  2. Music and reminiscence activities

  3. Gardening

  4. Cooking sessions

  5. Festive celebrations

  6. Wellness and interest-based activities

  7. Outings etc

  • Ensure activities are conducted in a safe, supportive, and inclusive environment that encourages residents’ participation and independence.

  • Monitor residents’ responses and participation and evaluate the effectiveness of programmes for continuous improvement.

  • Resident Engagement

  • Build positive relationships with residents and encourage them to participate in meaningful activities.

  • Work with nursing and allied health staff to identify residents’ interests, abilities, and social care needs.

  • Support residents during activities and outings to ensure a positive and enjoyable experience.

Outings & Community Engagement
  • Plan and coordinate outings, community visits, and external collaborative activities for residents.

  • Liaise with community partners, schools, volunteer groups, and corporate organisations to create enriching opportunities for residents.

Volunteer Management
  • Recruit, orientate, train, and coordinate volunteers to support social care programmes.

  • Maintain volunteer records, attendance, accreditation, and related documentation.

  • Build and sustain partnerships with community organisations, corporate partners, and volunteer groups.

Administrative & Ad-hoc Duties
  • Maintain accurate programme records, attendance, and evaluation reports.

  • Support departmental projects and community engagement initiatives as assigned.

  • Perform any other duties assigned by the supervisor or the COO’s Office.

Requirements
Qualifications
  • Diploma in any relevant disciplines.

  • Candidates with relevant experience in senior care, healthcare, social service, community programmes, or volunteer management are encouraged to apply.

Experience
  • 1–3 years of relevant working experience in programme planning, event coordination, volunteer management, or resident engagement is preferred.

  • Fresh graduates with a strong interest in senior care and community engagement are welcome to apply.

  • Organised and able to manage multiple activities and stakeholders.

  • Creative, resourceful, and proactive in planning engaging programmes.
