Recruit Express Pte Ltd is seeking a Community Social Services professional to coordinate care for clients at risk of mental health issues and dementia. You will plan outreach, conduct screenings, and develop intervention plans to promote wellbeing within the community.

The role involves regular client visits, assessments of walk-ins, and ongoing documentation and programme evaluation. A relevant diploma is required, with opportunities to contribute across diverse community settings in Singapore.

Qualifications

• Diploma in Social Service, Psychology, Counselling or related field.

Responsabilités

• Provide care coordination and service linkage to clients at risk of mental health issues and/or dementia.
• Plan and conduct community outreach to promote mental wellness.
• Plan and conduct case finding/screening to enable early identification.
• Organize preventive activities for at-risk groups and clients.
• Conduct regular visits to monitor clients' well-being.
• Assist in initial assessment of walk-ins and referrals; follow up with intervention plans.
• Involve in evaluation and documentation of programmes.
• Perform other duties as assigned by supervisor.

Min. Diploma in Social Service, Psychology, Counselling or any relevant field

Working Hours: 5 days office hours

Job Scope:

• Provide care coordination and service linkage to clients with or who are at risk of mental health issues and/or dementia, with the appropriate resources and support including caregiver support.
• Plan and conduct outreach to the community and stakeholders including faith-based, corporate and community organisations to promote mental wellness and mental health awareness.
• Plan and conduct targeted mental health and dementia case finding/screening to promote early identification and facilitate timely support.
• Plan and organize structured preventive activities and/or programmes for the community, those at risk of mental health conditions and clients.
• Conduct regular visits to monitor the well-being of the client
• Support in initial assessment of walk-ins and referral cases, follow up with intervention plans for clients as per the prescribed care plan.
• Involve in the evaluation and documentation of the programme.
• Any other duties as assigned by reporting officer or Head of Community Social Services
