Job summary

Recruit Express Pte Ltd. in Singapore is seeking a Senior / Social Worker for Day Care Centre and Active Aging Centre to deliver psychosocial assessments, intervention, and case management for clients with complex social and healthcare needs within the community.

You will collaborate with hospitals and community providers, advocate for client resources, mentor junior staff, and ensure high-quality documentation and service improvement through multidisciplinary teamwork.

Qualifications

• Min Bachelor Degree or Postgraduate qualification in Social Work recognised by SASW.

Responsibilities

• Conduct comprehensive psychosocial assessments for clients and caregivers.
• Develop and implement person-centred care plans based on clients' needs and goals.
• Provide counselling, emotional support, crisis intervention and practical assistance to clients and families.
• Man(age) complex and high-risk cases independently.
• Monitor clients' progress and review intervention plans regularly.
• Facilitate referrals and coordinate services with hospitals, community partners, government agencies and healthcare providers.
• Advocate for clients' needs and access to community resources and financial assistance schemes.
• Provide guidance, mentoring and clinical supervision to junior social workers, interns and support staff.
• Maintain accurate, timely and professional case documentation and reports.
• Participate in programme planning, service evaluation and quality improvement initiatives.

Working Hours: Mon-Fri, Office Hours

You are responsible for providing comprehensive psychosocial assessment, intervention, and case manment support to clients with complex social and healthcare needs within the community. The role supports clients and caregivers to -in-place safely and with dignity through coordination of care, advocacy, counselling, and multidisciplinary collaboration.

Job Scope:

Casework and Case Manment

• Conduct comprehensive psychosocial assessments for clients and caregivers.
• Develop and implement person-centred care plans based on clients' needs and goals.
• Provide counselling, emotional support, crisis intervention and practical assistance to clients and families.
• Man complex and high-risk cases independently.
• Monitor clients' progress and review intervention plans regularly.
• Facilitate referrals and coordinate services with hospitals, community partners, government ncies and healthcare providers.
• Advocate for clients' needs and access to community resources and financial assistance schemes.

Caregiver and Family Support

• Support caregivers in coping with caregiving stress and challenges.
• Conduct family meetings and mediate family related issues where necessary.
• Educate clients and families on available social and healthcare resources.

Multidisciplinary Collaboration

• Work closely with nurses, doctors, therapists, care coordinators and community partners to ensure integrated care delivery.
• Participate in case conferences, multidisciplinary meetings and care planning discussions.
• Build collaborative relationships with external stakeholders and service providers.

Leadership and Supervision

• Provide guidance, mentoring and clinical supervision to junior social workers, interns and support staff.
• Support onboarding and training of new staff.
• Contribute to team discussions and professional development initiatives.

Documentation and Administration

• Maintain accurate, timely and professional case documentation and reports.
• Ensure compliance with organisational policies, professional ethics and regulatory requirements.
• Prepare reports, statistics and service data as required.

Quality Improvement and Service Development

• Participate in programme planning, service evaluation and quality improvement initiatives.
• Contribute ideas to improve workflows, client outcomes and service delivery.
• Support audits and organisational projects where required.

Requirements:
