Job summary

celecti pte. ltd. is seeking Social Workers and Senior Social Workers in Singapore to deliver psychosocial support, counselling and case management for seniors and clients with healthcare needs.

Roles involve assessments, care planning, crisis intervention and collaboration with hospitals, government agencies and community partners. A Post Graduate Diploma in Social Work and SASW recognition are required, with at least 2 years of relevant experience.

Qualifications

• Post Graduate Diploma in Social Work recognised by the SASW.
• Registered Social Worker (RSW) or eligible for registration.
• Minimum 2 years of relevant social work experience.
• Experience in healthcare, eldercare, community care or case management is advantageous.

Responsibilities

• Conduct psychosocial assessments and develop person-centred care plans for clients and caregivers.
• Provide counselling, emotional support, crisis intervention and practical assistance to clients and families.
• Coordinate referrals and work with hospitals, healthcare providers, government agencies and community partners.
• Support clients and caregivers in accessing healthcare, social and financial assistance resources.
• Collaborate with multidisciplinary teams to deliver holistic care.
• Maintain accurate case documentation and prepare reports as required.
• Contribute to service improvement and professional development activities.
• Provide guidance and mentoring support to junior staff (for Senior Social Worker level).
