Job summary

CELECTI PTE. LTD. is seeking Social Workers and Senior Social Workers to join a leading provider of senior care and community-based healthcare services in Singapore.

The roles involve psychosocial assessments, counselling, and coordinating care with hospitals and community partners. 定位 South Central Singapore with travel to the West, 5-day work week, permanent full-time positions, and opportunities for senior roles based on experience.

Qualifications

• Degree or postgraduate qualification in Social Work recognised by SASW.
• Registered Social Worker (RSW) or eligible for registration is preferred.
• Min. 2 years of relevant social work experience, candidates with more experience may be considered for Senior positions.
• Experience in healthcare, eldercare, community care or case management is advantageous.
• Willing to travel to West Region when required.

Responsibilities

• Conduct psychosocial assessments and develop person-centred care plans for clients and caregivers.
• Provide counselling, emotional support, crisis intervention and practical assistance.
• Manage complex cases, monitor clients' progress and review intervention plans.
• Coordinate referrals and services with hospitals, healthcare providers, government agencies and community partners.
• Support caregivers and families through counselling, family meetings and education on available resources.
• Work closely with multidisciplinary teams to ensure holistic and coordinated care.
• Maintain accurate case documentation, reports and service records in accordance with professional and organisational requirements.
• Provide guidance and mentoring to junior social workers, interns and support staff where required.
• Participate in quality improvement, programme development, service evaluation and organisational projects.
