Job summary

NTUC Health Co-operative Ltd. is seeking a Client-Centric Care Coordinator to oversee home care services, discharge planning, monitoring and education for clients and families.

You will manage walk-in, online and phone inquiries, coordinate services, document accurately, and work with internal teams to ensure timely delivery of Home Personal Care. A Diploma in a health-related discipline is required, with Singapore experience and fluency in English plus another local language.

Qualifications

• Diploma or Advanced Diploma in a healthcare-related discipline.
• At least three years of experience in community healthcare settings in Singapore.
• Proficient in Google Workspace and Microsoft Office.
• Excellent customer service and communication skills.
• Fluent in English and at least one additional local language.
• Strong team player with the ability to multitask.
• Able to work independently and meet deadlines.
• Passion for community service and a service-oriented mindset.

Responsibilities

• Provide information and referral services to clients and families.
• Coordinate home care services and document the process.
• Conduct regular case management including home visits and follow-ups.
• Collaborate with internal teams and external partners on referrals.
• Assist in program development to maximize service utilization and client satisfaction.
