The Supreme HR Advisory Pte. Ltd. is seeking a dedicated Social Worker to join our team in Singapore.

You will lead intake screening, psychosocial assessments, and discharge planning for residents, coordinating with nursing staff and community resources to ensure comprehensive care across stages of admission and transition. Responsibilities include counselling, crisis intervention, and collaboration with multidisciplinary teams.

Formación

• Bachelor degree in Social Work or related field.
• Registered Social Worker with SWAAB.
• 3–5 years of relevant experience, hospital or community hospital setting preferred.
• Experience working with elderly and dementia.
• Advance Care Planning (ACP) facilitator certification is an advantage.

Responsabilidades

• Intake screening and admission support with nursing staff.
• Conduct psychosocial assessments of residents and families.
• Provide counselling and psychosocial interventions.
• Coordinate referrals to community resources and healthcare providers.
• Develop discharge and transition plans with the multidisciplinary team.
• Coordinate respite care referrals and care planning.
• Perform risk assessments and implement crisis intervention plans.
• Support End-of-Life discussions and bereavement for residents and families.
• Engage residents through group activities and community programmes.
• Participate in ongoing professional development and quality improvement.
