Based in Buangkok, Singapore, the position involves implementing programmes, preparing six-monthly review reports and monthly interview records for about 50 residents, and collaborating with care staff to ensure quality of life

Qualifications

• Degree in Social Work is required.
• Experience in Social Service Sector or related field preferred.

Responsibilities

• Develop individualised care plans for residents.
• Execute the care plan and monitor progress.
• Develop and implement programmes and activities to meet the residents' care needs.
• Put up 6th monthly review reports and monthly interview records for about 50 residents.
