Job summary

THE SUPREME HR ADVISORY PTE. LTD. invites applications for a Social Worker based in Buangkong. You will develop and implement care plans and programs to meet residents' needs, and monitor progress of each resident.

The role requires a Degree in Social Work; experience in social service is preferred. Working hours are Mon-Thu 9am-7pm and Fri 9am-6pm, with a supportive team environment and opportunities for professional growth.

Qualifications

• Degree in Social Work.
• Preferably experience in Social Service Sector or related field.

Responsibilities

• Develop individualised care plans for residents.
• Execute the care plan and monitor progress.
• Develop and implement programmes and activities to meet residents' care needs.
• Put up 6th monthly review reports and monthly interview records for about 50 residents.
