THE SUPREME HR ADVISORY PTE. LTD. is hiring a Social Worker to develop individualised care plans for residents and oversee their implementation in a Singapore-based facility.

You will execute care plans, monitor progress, and design programmes and activities to meet residents' care needs. Responsibilities include preparing six-monthly review reports and monthly interview records for about 50 residents.

Qualifications

• Degree in Social Work.
• Preferably experience in social service sector.
