Job summary

The Supreme HR Advisory Pte. Ltd. is recruiting a social care professional to develop and execute care plans for residents at a Singapore facility.

The role requires a Degree in Social Work and preferably experience in the Social Service sector. Salary ranges from SGD 3,500 to SGD 5,500 per month, with standard hours Mon-Thu 8am to 5:45pm and Friday 8am to 4:45pm, located in Sembawang.

Qualifications

• Degree in Social Work.
• Preferably experience in Social Service Sector or related field.

Responsibilities

• Develop individualise care plans for residents
• Execute the care plan and monitor progress
• Develop and implement programmes and activities to meet the residents’ care needs
• Put up 6th monthly review reports and monthly interview records for about 50 residents
