Job summary

The Supreme HR Advisory Pte. Ltd is seeking a dedicated Social Worker to support residents at Bukit Batok. You will develop individualised care plans, implement them, monitor progress, and coordinate care with nurses, family members, and other stakeholders to maintain high-quality support for residents.

Requirements include a Degree in Social Work and preferably experience in the Social Service sector. Working hours are Monday to Thursday 8am to 6pm and Friday 8am to 5pm; salary ranges from SGD

• Preferably experience in the Social Service Sector or related field.

Responsibilities

• Develop individualise care plans for residents.
• Execute the care plan and monitor progress.
• Coordinate care for residents with relevant stakeholder(s).
