Job summary

About the Role

Reporting tothe Assistant Director, Partnerships and Collaboration, the Position Holder will be responsible for the strategic development of portfolios to advance social-health integration and enhance the delivery of care in the community. This includes ideation and management of collaborations with partners and within NUHS to support the roll out of new policies and initiatives to augment NUHS’ role as the population health manager for the western region.

Requirements

• Ability to multi-task and work independently
• Ability to work in a fluid environment amidst multiple demands
