Job summary

DEMENTIA SINGAPORE LTD. in Singapore seeks a dedicated social care professional to conduct case assessments and develop individualized care plans for persons with dementia and their carers, while coordinating group sessions.

You will liaise with multidisciplinary teams and maintain accurate documentation, supporting outreach and collaboration with community partners. Occasional Saturday work and some weekday evenings are required.

Qualifications

• Degree in Social Work and Psychology or relevant discipline.
• Minimum 2 years of experience in community engagement or case management.
• Advance Care Planning conversation experience is a plus.
• Ability to work on Saturdays and some weekday evenings.
• Strong communication, organization and attention to detail.

Responsibilities

• Conduct case assessment and formulate Individualized Care Plans for persons with dementia and their carers.
• Plan and facilitate group sessions/activities for carers and/or persons with dementia.
• Provide interventions such as psychoeducation, supportive counselling and service linkages.

• Provide timely and accurate case recordings and documentation.
• Participate in team meetings and case conferences as required.
• Assist in data and statistics preparation for programme evaluation and reporting.
• Explore new avenues of collaboration with community partners and stakeholders.

• Perform ad-hoc tasks as required.

Requirements

• Requires work on scheduled Saturdays and occasional weekday evenings.
• Good oral, written and interpersonal communication skills.
• Ability to facilitate group sessions will be an added advantage.
