Villa Francis Home for the Aged in Singapore seeks a part-time administrative and case coordination support for the Social Work & Case Management team. You will coordinate resident admissions, liaise with residents, families, hospitals and service providers to ensure continuity of care.
The role requires a diploma or degree in related fields, strong interpersonal and organizational skills, and the ability to work in a multidisciplinary team to support seniors and their families.
Provide administrative and case coordination support to the Social Work & Case Management team.
Coordinate resident admissions, referrals and service arrangements with internal and external stakeholders.
Liaise with residents, family members, hospitals, community partners and service providers to facilitate care support and service follow-up.
Support care coordination activities, referral screening, reporting requirements and service transitions to ensure continuity of care.
Assist residents and families with applications for financial assistance schemes.
Prepare, submit and track referrals, applications and supporting documentation.
Monitor case-related appointments, referrals and follow-up actions to ensure timely completion.
Maintain accurate, timely and confidential records, reports and case documentation.
Participate in multidisciplinary meetings and support departmental programmes and initiatives.
Diploma or Degree in Social Work, Psychology, Counselling, Social Sciences, Nursing or a related discipline.
1 to 3 years of relevant experience in healthcare, community care, social services, case management or care coordination preferred.
Good interpersonal and communication skills.
Strong organisational and coordination abilities.
Able to work effectively in a multidisciplinary team environment.

Part-time: 3 to 4 days per week, office hours.
