National University Health System (NUHS) invites applications for a Programme Coordinator to support the NUHS@Home home hospital service in delivering coordinated patient care. The role focuses on planning, assessment, and monitoring of the program to ensure high-quality outcomes.

• Involve in the planning, assessment, and monitoring of our program to ensure that we are providing the best possible care to our patients
• Coordinate with external vendors to arrange for transport and courier deliveries
• Coordinate with NUH & cross hospital BMU to manage bed requests
• Coordinate with multi departments across NUHS to schedule appointments & other services as requested by the clinicians
• Perform patient admissions, discharges and transfers on Electronic Medical Record (EMR) system
• Conduct financial counselling to patient/NOK on the estimated NUHS@Home bills, available subsidies, various payment schemes and mode of payment.
• Assist patient to complete various modes of payment forms. Ensure proper documentation and follow up for all inflight case until patient discharge.
• Export reports from electronic medical records system and import data into relevant channels timely to facilitate information retrieval for the clinical team.
