Job summary

NUHS@Home is seeking a qualified nurse to provide at-home medical care, following physician orders and coordinating with the healthcare team in Singapore. You will assess patients, implement care plans, monitor progress, document outcomes, and educate families on self-care to ensure safe, continuous care.

You will supervise junior staff, participate in rounds, and support nursing administration with competency programs and training initiatives in a home-care setting.

Qualifications

  • Possess Diploma / Degree in Nursing and must be registered with Singapore Nursing Board
  • At least 1 year of post-registration clinical experience as Staff Nurse II
  • Ability to work independently and as a team
  • Strong organisational, coordination, interpersonal and communication skills
  • Ability to work a flexible schedule, including evenings, weekends and public holidays
  • Demonstrates competency in all core competencies and unit specific programs implemented by Nursing Administration

Responsibilities

  • Assesses patients’ needs on admission and daily throughout their hospital stay at home
  • Plans and implements care according to the assessed need
  • Evaluates and monitors patients’ progress according to planned care
  • Documents care given and monitors outcomes on the established charts and forms
  • Carries out treatments and investigations as prescribed by physicians
  • Communicates and discusses patients’ progress and response to treatment with physicians, nursing and allied health colleagues involved in the patient care
  • Follows-up to ensure continuity of patient care and treatment plans
  • Delivers safe and competent nursing care
  • Participates actively in physicians’ rounds
  • Prepares and assists physicians with procedures done on patients
  • Collaborates and co-operates with physicians and other health-care providers in the smooth delivery of patient care and services
  • Demonstrates clinical skills, knowledge and critical thinking in the delivery of patient care
  • Displays compassion, a positive and caring attitude towards patients as well as to internal and external clients
  • Provides patient/family information and education on disease prevention, health education and rehabilitation techniques
  • Demonstrates proper use and care of equipment
  • Demonstrates proper and economical usage of consumables and supplies
  • Work as a team in delivering care and interacting with healthcare team
  • Participates/involves in improving care and processes
  • Facilitate transition of care coordination within and between teams
  • Maintain a conducive environment for safe patient care and workplace safety
  • Use informatics, innovation and technology to manage nursing productivity and resource
  • Maintains a clean and safe working environment
  • Supervises and guides new/junior staff in their work as well as nursing students on posting
  • Assumes the role as Nurse in-charge

Job description

The NUHS@Home nurse provides nursing care to patients who require acute medical treatment but who are able to be cared for in their homes rather than in a hospital. The nurse works in collaboration with physicians and other healthcare professionals to develop and implement care plans, monitor patient progress, and educate patients and their families on self-care techniques.

The NUHS@Home nurse is competent in performing care assessment and planning, and in the management of patients with identified care needs. S/He provides clinical supervision, precepts the junior nurses and contributes to NUHS@Home nursing training.

Job Responsibilities
Primary Responsibilities and Duties (80%)
