ENVIRODYNAMICS SOLUTIONS PTE. LTD. is recruiting a Clinical Pharmacist for a leading public healthcare institution in Singapore.

The role accommodates work at Kent Ridge or West side polyclinics and focuses on delivering person-centered, team-based care across all stages of a patient’s health journey. The position involves reviewing orders for appropriateness, performing medication reconciliation, providing drug information to clinicians, participating in multidisciplinary rounds and ambulatory

Qualifications

• 5+ years' relevant experience as Clinical Pharmacist
• PharmD or equivalent degree recognized by the Singapore Pharmacy Council
• Experience in training and development of clinical curriculum/training programs
• Strong leadership and communication skills

Responsibilities

• Review written/electronic order forms to ensure prescribed medications are appropriate
• Perform medication reconciliation
• Provide drug information service to health care professionals
• Attend multidisciplinary rounds and ambulatory services; dispense and counsel patients
• Assigns, oversees and reviews the work of pharmacy assistants and technicians
• Train pharmacists to be advance generalists; craft learning objectives, curriculum and materials

Job Responsibilities:

• Review written/electronic order form (medication chart, medication record or prescription) to ensure drug prescribed is appropriate.
• Perform medication reconciliation.
• Provide drug information service to health care professionals.
• Involvement in clinical services such as attending multidisciplinary rounds and running ambulatory services. Dispenses and counsel patient on medication prescribed by doctor.
• Assigns, oversees and closely review the work of pharmacy assistants and technicians.
• Training pharmacists to be advance generalists specialists. Craft out necessary learning objectives, curriculum, materials.

Requirements:

• At least 5 years' relevant experience as Clinical Pharmacist
• PharmD or equivalent degree recognized by the Singapore Pharmacy Council
• Experience in training and development of clinical curriculum/training programs
• Strong leadership and communication skills
