Job summary

Woodlands Health is seeking a Pharmacist to review medication orders for safety and appropriateness, and to liaise with prescribers on interventions. The role includes dispensing medications, providing drug information, and supervising pharmacy staff to ensure legal compliance and accurate revenue recovery.

Qualifications

• Bachelor of Science degree in Pharmacy from an accredited University recognized by the Singapore Pharmacy Council.
• Registered with the Singapore Pharmacy Council and holds a valid practicing license.

Responsibilities

• Review each patient’s medication chart or prescription for the appropriateness of the drugs prescribed and to contact the prescribing doctor when an intervention is needed.
• Ensure that all orders are clear and specific and that doses are prescribed within therapeutic limits.
• Identify any potential adverse reactions and interactions and ensure accurate cost recovery.
• Dispense medications to patients, explaining indications for drug therapy and instructions for administration.
• Provide drug information and lectures to other healthcare professionals.
• Delegate, oversee and review the work of assigned subordinates; ensure compliance with legal requirements for medication sales and supply.
• Maintain records pertaining to all phases of drug receipt, storage and distribution as required by hospital or authorities.
• Represent the department in clinical committees and participate in pharmacy education and development programmes.
• Review pharmacy procedures and policies and implement improvements to workflow.
• Participate in national or institutional workgroups and projects.

MAJOR DUTIES & RESPONSIBILITIES

Specific

1. Review each patient's medication chart or prescription for the appropriateness of the drugs prescribed and to contact the prescribing doctor when an intervention is needed. The review has to ensure that:

5. Fulfil all drug orders accurately and in a timely manner.

6. Ensure that all related costs and revenue are accounted for, and accurately recovered.

7. Dispense medications to patients, explaining the indications for drug therapy and instructions for administration.

8. Provides drug information and lectures to other healthcare professionals.

9. Delegate, oversee and review the work of assigned subordinates.

10. Ensures that all sales and supply of medicinal products comply with legal requirements.

11. Records pertaining to all phases of drug receipt, supply, storage and distribution are maintained as required by the hospital and/or legal authorities.

12. Represent the department in various clinical committees and performs other job-related duties as assigned.

13. Review pharmacy procedures and policies periodically and implement schemes to improve pharmacy workflow.

14. Supervise performance, conduct appraisals, write reports and make recommendations for confirmation, promotion and training needs of subordinates.

15. Actively participate in Pharmacy education and development programmes.

16. Practices continued reading of journals, publications, etc for professional advancement.

17. Responsible for the implementation and adherence to policies, practices and regulations established by the department, hospital and external regulatory agencies pertaining to pharmacy.

18. Provide relevant training to pre-registration pharmacists and pharmacy students.

19. Support the hospital by participating actively in the various activities organized by the hospital. Activities include QC, community projects, staff welfare and recreational activities and etc.

Education / Training / Experience
