Responsibilities

• Build and maintain strong relationships with Community Care Partners to understand needs and aspirations
• Be the primary point of contact to support Partners with policies, schemes and regulations
• Support Partners on service management, operations, and implementation of initiatives
• Support capacity development and growth of Partners
• Drive improvements in operational processes and delivery of care services
• Coordinate with stakeholders to enable Partners' participation in programmes
• Maintain oversight of Partners’ implementation plan, progress and key information
