Job summary

SASCO SENIOR CITIZENS' HOME is seeking a Healthcare Assistant to provide compassionate elderly care in a sheltered home setting. You will work with a multidisciplinary team to assess and implement nursing care plans, identify signs of illness, and mentor HCAs in daily tasks. The role includes assisting with medications and managing common age-related conditions.

The position is based at Choa Chu Kang sheltered home with 44 hours per week, shift work, and a permanent role with benefits.

Qualifications

• Minimum GCE N Level
• Training in WSQ Higher/Advanced Certificate in Healthcare Support (Nursing Care) or ITE ATC Healthcare (Home Care) program
• Experience in elderly setting is an advantage
• Compassion, critical thinking and organizational skills

Responsibilities

• Comply with approved Nursing SOPs
• Work with multidisciplinary team to analyze and evaluate nursing care plan and ensure its implementation
• Identify symptoms of illness or decline and advocate early attention
• Mentor and guide HCAs in performing their functions
• Assist EN/RN in ensuring patients take medications
• Assist clients to manage age-related conditions such as incontinence and mobility issues
• Assist Enrolled Nurse/Registered Nurse as required
• Other duties as assigned by Centre Manager

Job purpose

• To provide optimal level of care in accordance to established policies and procedures.
• To assist in activities and programs promoting healthy aging for clients.
• Guide Healthcare Assistant on nursing related care to ensure optimal delivery of required care to the clients.

Duties and responsibilities

• Comply with approved Nursing SOPs
• Work with multidisciplinary team to analyze and evaluate nursing care plan and ensure care plan is implemented by team.
• Identify symptoms of clients related to illness or decline in functional/cognitive abilities and advocate early attention.
• Mentor and guide HCAs in performing their functional duties
• Assists the EN/RN in ensuring patients take their medications.
• Assist client to manage common age-related conditions such as incontinence and loss of mobility.
• Assist Enrolled Nurse/ Registered Nurse as when required.
• Other additional operational duties as assigned by Centre Manager.

Qualifications & Skills Required

• Minimum GCE ‘N’ Level
• Attained Training in either WSQ Higher Certificate/Advanced Certificate in Healthcare Support (Nursing Care); or Institute of Technical (ITE) Approved Training Centre (ATC) Healthcare (Home Care) program.
• Experience working in an elderly setting is an advantage
