Qualifications

• Diploma in Nursing or equivalent.
• Experience in clinical nursing care planning.
• Ability to collaborate effectively in a multi-disciplinary team.

Responsibilities

• Provide and coordinate nursing care for patients with relatively lower acuity.
• Collect relevant clinical and psychosocial data for assigned patients.
• Apply clinical reasoning to identify health conditions, needs and risks; escalate when needed.
• Set measurable short- and long-term goals with patients and plan preventive measures.
• Implement care plans and evaluate effectiveness.
• Communicate effectively to coordinate nursing care.
• Prepare for timely discharge and ensure continuity of care post-discharge.
• Educate patients and families with basic care needs.
