Job summary

You will collect clinical data, apply nursing reasoning, set goals with patients, implement care plans, educate families, and assist with discharge planning while adhering to professional, legal and ethical standards.

Qualifications

• Registered nurse with Diploma in Nursing.
• Career Conversion Programme for Registered Nurses (Diploma).

Responsibilities

• Provide and coordinate general nursing care for patients in the team.
• Collect relevant clinical and psychosocial data about assigned patients.
• Apply clinical reasoning and escalate as needed.
• Set measurable goals with patients and plan preventive/interventions.
• Implement care plan and evaluate outcomes.
• Communicate effectively to coordinate nursing care.
• Prepare for timely discharge and ensure continuity of care.
• Educate patients and families on care needs.
• Participate in multidisciplinary team planning and documentation.
• Lead junior staff and maintain safety standards.
• Pursue professional development and patient safety initiatives.
• Attend to general duties as assigned.
