Job summary

RN Care Pte. Ltd. in Singapore seeks a registered nurse to perform biopsychosocial and environmental assessments, plan and evaluate person-centred nursing care, and communicate plans with patients and families.

The role covers wound care, medication management, and continence care while upholding patient dignity, safety and confidentiality. Collaboration with multidisciplinary teams and emergency protocol initiation are expected.

Qualifications

• Must have a Diploma in Nursing.
• Registered with the Singapore Nursing Board.
• Minimum 3 years' experience in acute and/or community hospital/care setting.

Responsibilities

• Perform biopsychosocial and environmental assessments to identify strengths and needs of clients, families and/or caregivers.
• Plan, develop and evaluate person-centred nursing care plans and recommend modifications according to changing needs.
• Communicate care plans and address concerns from patient and family.
• Implement and evaluate care interventions (e.g., wound care, medication management, continence care) with emphasis on safety, dignity and confidentiality.
• Manage patient workload through timely planning and scheduling for designated groups.
• Build therapeutic relationships to empower clients and caregivers for self-management.
