Job summary

WECRUIT PTE. LTD. is seeking a Registered Nurse (Home Care) to deliver safe and high-quality nursing services at patients' homes in Singapore.

You will assess patients, develop and implement care plans, and collaborate with physicians and a multidisciplinary team to optimise outcomes. The role requires SNB registration, a nursing diploma, and post-registration experience in hospital, hospice, or community care settings.

Qualifications

• Diploma in Nursing and SNB registration.
• Post-registration nursing experience in hospital, hospice, or community care.
• Proficient in basic IT systems and electronic documentation.
• Detail-oriented with strong organisational and time management skills.
• Good interpersonal and communication skills with ability to work in a multi-disciplinary team.
• Able to work under pressure in a fast-paced healthcare environment.

Responsibilities

• Provide safe, competent nursing care with strong documentation and infection control.
• Manage complex patient care using clinical knowledge and critical thinking.
• Assess and monitor patients; develop care plans and support families.
• Coordinate care with physicians and multidisciplinary teams.
• Build relationships with patients and caregivers; assess home care needs.
• Ensure continuity of care; follow up on treatment plans.
• Perform nursing procedures: medication administration, wound care, and other clinical interventions.
• Maintain accurate documentation of patient conditions, referrals, enquiries, and attendance.
• Participate in multidisciplinary meetings to ensure seamless care.
• Promote patient safety by adhering to guidelines and policies.
• Ensure proper storage of medications and supplies; comply with regulations.
• Maintain confidentiality and handle sensitive information appropriately.
• Provide professional support and respond to enquiries from patients and families.
• Comply with SNB standards and regulatory requirements.
• Stay updated with industry guidelines and continue clinical development.
• Maintain a clean, organized, and safe working environment.
