Job summary

National University Polyclinics seeks a Manager, Transitional Care and Community Partnership to lead community health initiatives for TGCH. You will translate strategy into actionable programmes, work with residents and partner organisations, and build capacity to support hospital opening.

Responsibilities include operationalising the Community-Based Hub, developing governance processes, and supervising staff while maintaining cross-sector collaboration in a fast-paced environment.

Qualifications

• Bachelor's degree in Community Development, Public Health, Social Work, or a related field.
• At least 8 years of relevant experience in community health or development, with 3+ years in supervision.
• Experience in programme implementation and operational management in health/community settings.
• Familiarity with Asset-Based Community Development principles.
• Ability to coordinate across sectors and manage complexity in health and social care settings.
• Strong analytical, communication and presentation skills.

Responsibilities

• Lead community engagement and build local capacity through ABCD-inspired programmes.
• Develop and manage cross-sector partnerships with health, social and community organisations.
• Operationalise the Community-Based Hub with governance and workflows.
• Oversee project delivery, supervise junior staff, and ensure departmental governance.
• Act as deputy to Head of TCCP and maintain operational continuity.
