Job summary

GMP Technologies in Singapore seeks a Senior Assistant Manager / Assistant Manager for Transitional Care and Community Partnership. Qualifications

• 3-5 years of work experience in population health, care integration or community development settings.
• Familiarity with strengths-based approaches and participatory methods.
• Ability to work across sectors and navigate complexity in health and social care settings.
• Strong communication, facilitation and relationship-building skills.

Responsibilities

• Build trust-based relationships with residents and local stakeholders to identify community strengths and priorities.
• Conduct placed-based outreach to understand individual and community aspirations using lived experience and data.
• Facilitate community-led efforts that promote social connection and neighbourhood wellbeing.
• Support residents in navigating health and social systems to enable independence and resilience.
• Maintain active partnerships with hospital and community agencies to coordinate care.
• Prepare periodic reports for Hospital Committee and assist with other project tasks.
