Job summary

National University Health System (NUHS) seeks a Manager, Transitional Care and Community Partnership for Tengah General and Community Hospital. You will translate strategic community health directions into work plans, programmes and partnerships, collaborating with residents, community groups and partners to activate local assets.

You will oversee day-to-day operations, supervise junior staff, and build processes to support TGCH’s opening, applying Asset-Based Community Development principles

Qualifications

• Bachelor's degree required in a related field.
• 8+ years of relevant experience in community health, care integration or community development.
• At least 3 years in a supervisory or team lead capacity.

Responsibilities

• Operationalise community engagement strategy and activities.
• Develop and manage multi-sector partnerships and collaboration.
• Lead planning and activation of the Community-Based Hub and governance processes.
• Supervise staff, manage team performance, and contribute to governance and planning.
