Responsibilities

• Assess patients' needs on admission and during home visits.
• Plan and implement care according to assessed needs.
• Evaluate and monitor patients' progress per care plan.
• Document care and monitor outcomes on charts/forms.
• Carry out prescribed treatments and investigations.
• Communicate patient progress with physicians and care team.
• Ensure continuity of patient care and treatment plans.
• Deliver safe and competent nursing care.
• Participate in physicians' rounds and assist procedures.
• Collaborate with physicians and other healthcare providers.
• Demonstrate clinical skills, knowledge and critical thinking.
• Provide patient/family education on health and rehabilitation.
• Maintain safe, clean working environment and proper equipment usage.
• Supervise junior staff and nursing students.
