Job summary

Ren Ci Hospital in Singapore is seeking an Occupational Therapist to conduct assessments and develop individualized treatment plans to improve functional independence and quality of life for patients in community and home settings.

You will perform home safety assessments, prescribe assistive devices, provide caregiver training, and coordinate with multidisciplinary teams to ensure seamless discharge planning and continuity of care.

Qualifications

• Degree in Occupational Therapy recognised by MOH/AHPC.
• AHPC registration and Practising Certificate required.
• Experience in community, rehabilitation or home care settings preferred.
• Strong clinical assessment, treatment planning and problem-solving skills.
• Ability to work independently in patients' homes and community settings.

Responsibilities

• Conduct occupational therapy assessments and develop individualized treatment plans to improve patients' functional independence and quality of life.
• Perform home environment assessments to identify safety, accessibility, and fall risks.
• Recommend and prescribe assistive devices, adaptive equipment, and home modifications to support ageing-in-place.
• Evaluate treatment outcomes and modify interventions according to patients' changing needs.
• Collaborate with multidisciplinary teams, patients, caregivers, and external agencies to ensure coordinated care and discharge planning.
• Educate, train, and support patients and caregivers in managing daily living activities safely and effectively.
• Maintain accurate, timely, and confidential clinical documentation and patient records.
• Supervise therapy aides and support the supervision of occupational therapy students where required.
• Participate in professional development, departmental meetings, training, quality improvement initiatives, and research-related activities.
• Liaise with community partners to facilitate continuity of care.
