ACTIVE GLOBAL RESPITE CARE PTE. LTD. is seeking a Home Care Coordinator to screen referrals, develop care plans, and coordinate services. You will conduct home visits, ensure adherence to care plans, and liaise with doctors, nurses, therapists, and social workers to meet clients’ needs.

The role involves significant fieldwork (approximately 70% of duties) and requires empathy, strong listening skills, and the ability to adapt in a dynamic healthcare setting.

Formación

• Diploma or bachelor’s degree in any field.
• Work experience in healthcare or social service preferable.

Responsabilidades

• Screen and review IC referrals for medical, functional, and cognitive conditions and identify client care needs.
• Ensure ICs care staff follow the referral care plan and client needs.
• Follow up with ICs clients to ensure long-term care plan.
• Conduct home and hospital visits; about 70% of time.
• Refer clients or caregivers to relevant services when needed.
• Raise referrals via BRIGHT; submit appeals or extensions.
• Assess and submit Household Means Testing applications.
• Conduct interRAI assessments and develop individualised care plans.
• Collaborate with referral sources on discharge plan.
• Coordinate client care with doctors, nurses, therapists, MSWs, and HCAs.
• Advise clients on financial scheme applications.
• Handle client and referral source inquiries.
• Assist with audit-related tasks as required.
• Handle ad hoc tasks assigned by Head of Home Care.

• Screen and review all ICs referrals for medical, functional, and cognitive conditions, and identify the client's care needs if the referral source has not already done so.
• Ensure ICs care staff provide care according to the referral source's care plan and the client's needs.
• Follow-up with ICs clients midway through the service as and when required to ensure they have a longterm care plan.
• Conduct home and hospital visits for potential clients; these visits will account for about 70% of the time.
• Where necessary and applicable, refer the client and/or caregiver to relevant services or healthcare providers.
• Raise referrals for eligible clients via BRIGHT to required services; submit appeals or extensions where applicable.
• Assess and submit Household Means Testing application for clients.
• Conduct interRAI assessment and develop an individualised care plan.
• Work with referral sources on the client's discharge care plan.
• Coordinate client care with doctors, nurses, therapists, MSWs, and HCAs within the company.
