Job summary

RECRUIT EXPRESS PTE LTD is seeking a qualified nurse to provide in-home and community-based palliative care. You will perform comprehensive assessments, develop care plans, and collaborate with a multidisciplinary team to deliver compassionate care.

Applicants must be SNB-registered with BCLS, and have palliative care experience. The role requires strong communication, independence, and patient-centered approach in home settings.

Qualifications

• Registered with the Singapore Nursing Board (SNB) and valid BCLS certification.
• Relevant palliative care experience is required.
• Relevant palliative care certification / qualification is required.
• Strong communication and interpersonal skills.
• Compassionate, patient-centred approach; able to work independently.

Responsibilities

• Provide holistic and compassionate nursing care at home and for palliative patients.
• Do nursing assessments and monitor physical and psychosocial conditions.
• Develop and implement nursing care plans based on patient needs.
• Manage symptoms and provide palliative nursing interventions.
• Administer meds and treatments per orders and protocols.
• Provide wound care and pain management as required.
• Educate patients and families on disease management and end-of-life care.
• Collaborate with doctors, allied health professionals and caregivers.
• Offer emotional and psychosocial support to patients and families.
• Maintain accurate nursing documentation and records.
• Participate in care coordination and follow-up; ensure continuity of care.
• Adhere to nursing standards, infection control, and organisational policies.
• Perform other duties as assigned.
