Job summary

The role involves independent practice, caregiver training, and collaboration with healthcare teams in a home setting. The successful candidate will hold the SNB Practising Certificate, have at least four years of nursing experience, and be willing to provide after-hours

Qualifications

• Registered with the Singapore Nursing Board (SNB) with a valid practising certificate.
• Minimum 4 years of ongoing nursing experience in medical or geriatric care within the past 2 years.
• Experience in oncology or palliative care is advantageous.

Responsibilities

• Provide specialised home-based nursing care and clinical interventions.
• Conduct initial nursing assessments and six-monthly reviews; develop care plans.
• Educate and train caregivers on nursing procedures, safety and infection control.
