Job summary

RECRUIT EXPRESS PTE LTD is seeking a dedicated registered nurse to provide home and palliative care services to patients in the community. You will perform assessments, develop care plans, administer meds, and support patients and families.

The role requires SNB registration, valid BCLS, and experience in pallliative care. You will collaborate with doctors and a multidisciplinary team to ensure compassionate, patient-centred care in home settings.

Qualifications

• Registered with the Singapore Nursing Board (SNB).
• Valid BCLS certification.
• Relevant palliative care experience is required.
• Relevant palliative care certification/qualification is required.
• Strong communication and interpersonal skills.

Responsibilities

• Provide holistic and compassionate nursing care to patients receiving home care and palliative care services.
• Conduct nursing assessments and monitor patients' physical and psychosocial conditions.
• Develop and implement appropriate nursing care plans based on patients' needs.
• Manage symptoms and provide appropriate palliative nursing interventions.
• Administer medications and treatments in accordance with doctors' orders and established protocols.
• Provide wound care, pain management, and other nursing procedures where required.
• Educate patients, caregivers, and family members on disease management, medication, and end-of-life care.
• Work closely with doctors, allied health professionals, caregivers, and other members of the multidisciplinary team.
• Provide emotional and psychosocial support to patients and their families.
• Maintain accurate and timely nursing documentation and patient records.
• Participate in care coordination and follow-up to ensure continuity of care.
• Adhere to professional nursing standards, infection control procedures, and organisational policies.
• Perform other duties as assigned.
