Job summary

RS RECRUITMENT AGENCY LLP in Singapore seeks a Social Worker to provide professional, ethical, and person-centred social work services in a long-term care setting. You will conduct comprehensive assessments, develop care plans, and collaborate with a multidisciplinary team to support residents and families.

You should hold a recognized degree in social work (Bachelor/Master/Graduate Diploma), be SWAAB-registered, SASW member, and have at least two years’ experience in hospitals or nursing homes.

Qualifications

• Hold a recognized Bachelor's Degree, Graduate Diploma, or Master's in Social Work from a SWAAB-accredited institution.
• Be a registered social worker with the SWAAB.
• Member of the SASW.

Responsibilities

• Conduct comprehensive social, psychosocial and BPSS assessments to identify needs, risks and support systems.
• Develop, implement and review individualised case management plans and interventions.
• Provide counselling, emotional support and relevant information to residents, family members, caregivers and significant others.
• Maintain accurate, confidential documentation of assessments, plans and progress.
