Job summary

Rainbow Center Singapore is seeking a dedicated Social Worker to support persons with disabilities and their families through person-centred planning and coordinated care. The role emphasizes building strengths, advocacy, and connecting clients with services to live as independently as possible.

You will conduct needs assessments, provide counselling, crisis intervention, and collaborate with families, schools and community partners to strengthen the circles of care around our clients.

Qualifications

• Bachelor or Master of Social Work degree from a recognized university.
• Accredited Social Worker, or eligible for professional registration with the SASW.
• Experience working in a similar setting or with persons with disabilities is advantageous.

Responsibilities

• Conduct assessments of client needs, provide counselling, support and crisis intervention.
• Coordinate care with families, healthcare providers and community resources to ensure comprehensive support.
• Participate in programme and policy development and work with families and community agencies to strengthen circles of care around clients.
