National University Hospital is seeking a Case Management Officer (Psychological Medicine) for a 1-year contract. You will assess, plan, coordinate and monitor cases, coordinating with doctors, psychologists, nurses, social workers and other allied health staff to ensure continuity of mental health care for patients in our ASCAT/AFTERCARE program.

Qualificações

• Bachelor degree preferably in Nursing, Biomedical Sciences, Psychology or Healthcare
• Minimum 4 years of experience in managing mental health conditions including 2 years in a hospital
• Prior work experience in psychiatric facilities (inpatient/outpatient) is advantageous
• Nursing background with advanced/postgraduate diploma in mental health is advantageous
• Social work background helpful
• Strong organizational, interpersonal and communication skills
• Ability to communicate in local languages and dialects
• Ability to multi-task and work independently and as part of a team

Responsabilidades

• Support clinicians assigned to plan, execute and monitor care plans promptly including collaboration with community partners and educational institutions
• Provide prompt progress updates to attending physicians and consult supervisors as needed
• Follow up with patient/community partner queries within same day for urgent cases and within 3 working days for normal cases
• Provide accurate documentation in Epic and patient databases and to attending physicians
• Handle patient complaints and feedback
• Maintain project databases and KPIs
• Accept referrals to outpatient clinic from GP/Polyclinic and community partners
• Provide transitional care/discharge to community partners/GPs
• Participate in case discussions with community partners
• Engage with community/GP partners and assist in recruitment and partnerships
• Contribute to planning engagement meetings with new community partners
• Collaborate with ASCAT/AFTERCARE team to meet clinical KPIs
